- Doctor name, email ID and phone number
- Specialisation and registration details
- Clinic or hospital name and full address (city, state, PIN code)
- GST number (optional)
- A photo of your clinic (optional)

Email the details above to support@hisgro.com with the subject line "Doctor registration", or use our contact form and choose "Doctor registration" as the enquiry.
